The data controller is DENTAL BEAUTY DESIGN S.R.L., registration number CUI 42371357, registered at Bd. Alexandru Lăpușneanu 106, room 1, block L1, staircase B, ground floor, apartment 24, Constanța, Romania. Contact: dentalbeautydesign@gmail.com.

We use your name, telephone number, location, service and preferred day and part of the day to respond to your request and arrange a consultation. Submission does not reserve a time slot. Do not send diagnoses, medical documents or payment details. Providing information is optional, but without a name and telephone number we cannot respond through this form. You may call the clinic directly.

Processing supports steps you request before receiving a service, under GDPR Article 6(1)(b). Health information, where involved, is handled under Article 9(2)(h) and professional confidentiality. We do not use requests for marketing or automated decisions.

Access is limited to authorised clinic staff. GATA MÂINE and hosting provider Hostico provide technical support. Notifications are sent to the clinic’s Gmail address; this service may involve processing outside the EEA under the safeguards described in Google’s privacy policy. Application data is stored separately from public files on the hosting server.

We retain a request for as long as necessary to contact you and resolve it, and afterwards only as needed to meet legal obligations or establish, exercise or defend a legal claim. Medical documents arising from a consultation are managed separately from this form. Contact the clinic to request deletion or clarify the retention period applicable to your request.

You may request access, correction, deletion, restriction and, where applicable, portability or object to processing. You may lodge a complaint with the Romanian supervisory authority, ANSPDCP. Address requests to the clinic at the email above. Last updated: 7 November 2026.
